Upon receipt of an appeal and request for a hearing, the county agency is required to complete an “Appeal Background Report,” SFN 1784, and send to the Appeals Supervisor. Along with this form include all pertinent documentation such as notices sent to the recipient; the written appeal request and verification provided by the recipient.
